[1] Gordon et al (2007) conducted a study which aimed to identify medication-related problems and identify approaches to support patients in managing their medications. Problems relating to access and organization of services were one of the findings highlighted by the study. The pharmacy can address issues associated with the organization of services, patient education and the involvement of patients in the decision-making process on medicines to be taken at home during the consultation. Continuing care is the transition of a patient from the hospital to home. It also includes the care provided to a patient in the healthcare system across the full spectrum of healthcare providers and their environments. It is carried out without interruption. The pharmacy can contribute so that when one pharmacist ceases to be responsible for patient care, another must take over.
