HNC Graduated Health Care Unit - Planning I am currently in a nursing home that provides long-term care for the elderly. Following the Data Protection Act 1998, in order to maintain confidentiality, for the purposes of this activity the resident will be referred to as Grace. After evaluating a number of residents' needs, I came to the conclusion that this person would benefit most from this activity. I will explain the reasons why below. Grace is an 87-year-old female resident with a current diagnosis of mixed dementia. Grace's diagnosis of mixed dementia has resulted in a decline in her cognitive abilities, particularly her ability to remember. This also resulted in Grace being considered a vulnerable adult and being placed under an Adults with Incapacity Act 2000 declaration. In order for Grace to be treated with a holistic, person-centred care approach, I will work with Grace to develop a scrapbook containing pictures and stories about her life. The aims of the activity are; engage Grace in a mentally stimulating activity that provides for all dimensions of her health and needs; Physical, psychological, social, emotional and spiritual. (WHO, 2011) The care approach used by nursing homes is a person-centred care plan, which allows all dimensions of health to be catered for. To meet Grace's physiological health needs it is important to first identify her physiological health problems, these problems include; a diagnosis of mixed dementia and a previous medical history of hypertension, hiatal hernia, anemia, and recurrent urinary tract infections. Due to dementia, Grace is prone to agitation (for which she receives 50mg Trazadone, 3 times a day), however this agitation is reduced by music, so I will accommodate this need and facilitate background music to reduce anxiety.
